I have a collection of medical forms, lab reports, and diagnostic summaries that are already in Word documents, Excel sheets, and a handful of other digital formats. I need every clinically relevant detail lifted from those source files and entered into a single, standardized spreadsheet template that I will share with you at kickoff. You will open each file, copy the data field by field exactly as it appears, and verify that patient identifiers, dates, CPT or ICD codes, and result values carry over without error. When a value is unclear or missing, flag it in a separate column so I can resolve it later. Because this is protected health information, you must handle the files in a secure environment and agree to keep them strictly confidential. Familiarity with medical terminology and a careful eye for small discrepancies are essential—accuracy is more important to me than speed. Deliverables: • Completed master spreadsheet populated with 100 % of the source data • A brief log of any ambiguities, illegible items, or missing values you discovered • Confirmation that all files were processed and stored using the naming convention I provide If you normally work with Excel functions, data validation, or have experience importing into EMR/EHR systems, please mention it when you respond.
